Fees

The club/organization fee is $75.00 (there is a $100 late fee for renewal applications received after May 1 from competitive clubs).

Required for clubs conducting scheduled club practices

	Club practice insurance information & practice registration form (PDF) Each coach/volunteer must submit ONE of the following forms to be covered by the USATF liability insurance policy during club practices
	Youth Club: Coach/Volunteer Information Form $ Background Check Acknowledgement (PDF) (for ALL coaches/volunteers who have direct contact with youth 17 & under during club practices/activities)
	Adult Coach/Volunteer Registration Form (PDF) (for coaches/volunteers who supervise club practices/activities where ALL participants are 18 years or older)

Background Checks Process

Effective 2009, the background check is handled via a centralized process. All coaches and volunteers of youth clubs are required to submit to a background check. The fee is $8.00 per application.
